New York State has a health improvement plan called the Prevention Agenda, which serves as the blueprint for state and local action to improve the health and well-being of all New Yorkers and to promote health equity in all populations who experience disparities.
Every three years, county health departments, the hospitals in the county and community partners are tasked with assessing community health needs and then creating a plan to address the needs and work to improve the health of county residents. The team in Steuben is called Smart Steuben and has worked together for more than a decade.
